Recent changes to federal law under the Employer Reporting Improvement Act have updated how employers are required to distribute Form 1095-C (Employer-Provided Health Insurance Offer and Coverage).
Our organization will continue to prepare and file Forms 1094-C and 1095-C with the Internal Revenue Service as required.
Employers are no longer required to automatically distribute Form 1095-C unless a copy is requested.
Current and former employees may request a copy of their Form 1095-C at any time, and one will be provided promptly.

Requested forms will be provided within 30 days of the request.
Form 1095-C is not required to file your individual income tax return, but it may be helpful for your personal records.
